Forms can be completed at office visits at no charge. All children will be given an updated 3231 form, the immunization form needed for all daycares and schools , every time an immunization is given. Sports forms, camp forms, and school forms can be filled out at a checkup visit for no additional charge. Please be sure to fill out your part ahead of time. Due to the amount of administrative time required, there is a $10.00 fee for all forms , including immunization forms, requested at other times. See the full form policy here. |

Once your newborn leaves the hospital if there is ever a reason for a hospital admission it would be at Children’s Healthcare of Atlanta at Egleston or Scottish Rite. Both hospitals have a team of doctors, called Hospitalists, who are available 24/7 and will take excellent care of your child during the hospital stay. There is ongoing communication with us, a discharge summary is sent, and all follow up is done in our office. |

We are on staff at Dekalb Medical Center and at Piedmont Hospital, and if selected by you we will come to check your new baby within 24 hours of delivery. We will examine the baby, review the chart and come to your room to go over the exam and talk about new baby issues, such as feeding, cord care, and jaundice. If your baby has any unexpected complications both hospitals have exceptional neonatologists who will care for your baby and coordinate with us as you are ready for discharge. Babies born at other area hospitals are seen by physicians on staff there, and then seen in our office a few days after discharge. Read more about newborns here. |

Our office hours are Monday through Friday 8:30am unti 5:00pm. The phones are “off”, except for emergencies from 1:00 until 2:00pm. All calls are answered by our front office staff who will assist you. Same day sick appointments are available every day, nurse visits for things like newborn weight checks, immunizations, and strep tests for older children are available daily, and well visits are scheduled up to three months in advance. Our nurse is available free of charge from 9:00 to 5:00 to help you with routine advice, help decide if an office visit is necessary, and process prescription refills. At times, at your request, with physician guidance certain medical conditions can be handled safely over the phone, saving you an office visit.
